Visiting a North Sea installation for what was initially thought to be a routine wellbeing check on behalf of a regional operator, our team instead uncovered a more severe situation.

The challenge
Engaging with the workforce on board the installation provided our team with a clear outline of the underlying cultural difficulties that had been building underneath the surface. The asset was showing early signs of a breakdown in psychological safety, demonstrated by:

Rapid Onboard Support: We stabilised and supported the safety-critical worker immediately, ensuring safe operations and providing trauma-informed strategies to regain control and focus.
Confidential Onshore–Offshore Link: Multiple personnel came forward (many for the first time) to discuss bullying, stress, anxiety and fatigue. We offered a psychologically safe space where they were heard, supported, and guided.
Escalation With Purpose: Leadership concerns were escalated anonymously, enabling the operator to begin addressing toxic behaviours without exposing staff.
Clear, Actionable Insights: We delivered practical recommendations on fatigue management, communication, leadership behaviour and ongoing psychological support.

Working in high-pressure offshore environments for long periods can be isolating and challenging for even the most resilient people. Without a specialised and private space for personnel to disclose concerns and anxieties without fear of judgement or news spreading, it can be tricky to know and understand what is going through someone's head.
During a routine rotation on a major offshore installation, a staff member requested a confidential session with an IntrospeXion psychologist. What followed became one of the most significant interventions we’ve delivered.
The challenge
After spending time with our on-site psychologist, the individual expressed clear indicators of suicidal ideation. Signs included:
Intense feelings of hopelessness.
Severe emotional distress linked to personal and work triggers.

This was a high-risk scenario in one of the most isolated environments possible. With limited medical resources offshore and no immediate access to care, the situation required urgent, expert intervention.
Our approach
IntrospeXion’s offshore-certified psychologist conducted an immediate risk assessment, followed by a structured safety plan.
Rapid : The individual was seen within minutes of raising concerns, no waiting lists, no delays, no barriers.
Clinical Risk Assessment: A full psychological evaluation was carried out to identify intention, means, protective factors, and immediate triggers.
Crisis Containment & Emotional Stabilisation: We used evidence-based techniques to de-escalate intense emotional distress and bring the individual back to a safer psychological state.
Coordination With Leadership (Confidentially and Ethically): We worked discreetly with the OIM to ensure the worker was not left alone; their safety was continuously monitored; and the situation was handled with dignity and privacy.
Safe Repatriation: A structured and compassionate handover was coordinated with the OIM, medic and onshore duty-holder to ensure continuity of care upon return.
The Transformation
After consultation and once back onshore, the individual continued receiving psychological support through IntrospeXion and a trusted Employee Assistance Program (EAP). Within weeks, the individual reported feelings of:

Stronger coping strategies.
Reduced suicidal thoughts.
Renewed confidence to return to work safely when appropriate.
